[摘要]
目的: 使用专用内提升器械进行上颌窦不同高度的上颌窦内提升,评价在不植骨的情况下同期植入
不同植体的初期稳定性。方法: 52 例( 89 枚) 上颌后牙高度不足病人,根据上颌窦底剩余牙槽骨高度( Residual
Bone Height,RBH) ,分为两组: A 组45 例,3 mm≤RBH≤5 mm; B 组44 例,5 mm≤RBH≤10 mm。根据种植体类
型分为RN 组、TE 组。采用单纯上颌窦内提升术( Osteotome Sinus Floor Elevation,OSFE) 同期植入瑞士
Straumann种植体RN 4.1mm×10 mm、RN 4.1mm×8 mm、TE 4.1mm×10 mm 及TE 4.1mm×8 mm 种植体,分别测量
种植体近中、远中、颊侧、舌侧的稳定系数,取平均值记录为该种植体的初期稳定系数,比较两组种植体稳定系数
在植入后有无差异。结果: A、B 两组种植体的初期稳定性有显著性差异。RBH 范围在3 ~ 10 mm 时,TE 与RN
种植体植入的初期稳定性有显著性差异。无论采取级差备洞还是逐级备洞,TE 与RN 种植体的初期稳定性有
显著性差异。结论: 运用冲压法上颌窦底提升术植入的种植体的初期稳定性随着BRH 的减少而降低,但RBH
范围在3~ 10 mm 时,种植体的初期稳定性是可预期的。同时在此范围内,TE 种植体植入的初期稳定性均较与
RN 种植体稳定性为高,都达到了较好的初期稳定性。TE 种植体结合级差备洞更易获得种植体的初期稳定性。

[Abstract]
Objective: To evaluate the primary stability of immediately inserted different dental implants
with Osteotome Sinus Floor Elevation ( OSFE ) without grafting. Methods: 52 patients ( 89
implants) had severely resorbed alveolar bone in posterior maxillary region divided into two groups on
the basis of the residual bone height( RBH) of the maxillary sinus,Group A: 45 cases with the RBH between
3 mm and 5 mm.Group B: 45 cases with the RBH between 5 mm and 10 mm.We also divided into an RN、TE group depending on the type of Swiss Straumann implants, including RN 4.1mm×10
mm,RN 4.1mm×8 mm,TE 4.1mm×10 mm and TE 4.1mm×8 mm.Measure and record on average the
implant stability coefficient( ISQ) of plants in the mesial,distal,buccal and lingual, then compared the
differences of two groups.Results: There was significant difference of initial stability between group A
and B.When the RBH range 3~10 mm in posterior maxilla arear,TE and RN implants have significant
difference.Either differential backup group or traditional implanting group, there were also significant
difference between TE and RN implants.Conclusion: With the reducing of the residual bone height, the
primary stability of dental implants was also decreased when using stamping method.But the stability of
implantation can be expected when RBH in posterior maxilla arear is between 3 mm and 10 mm.At the
same time in this range, the primary stability of TE implants were higher than RN ones. TE implant
combined with differential backup are more likely to get the primary stability.
