[摘要]
目的: 探讨超声造影在鉴别甲状腺良恶性结节中的应用价值。方法: 选取2014-09 ~ 2015-03 在我
院接受检查的78 例病人的95 个甲状腺结节的超声常规图像及造影图像,进行回顾性分析,观察良恶性结节的
增强均匀性、有无环形增强等特点,按照结节大小、结节内部回声分组,观察结节增强特点。结果: 95 个结节中,
42 个恶性结节,53 个良性结节,良恶性结节超声造影增强均匀性、有无环形增强差异有统计学意义( P = 0.000) 。
以≤1cm 与>1cm 结节分组,良恶性结节比较差异有统计学意义( P = 0.000) ; 以囊实性及实性结节分组,良恶性
结节比较差异有统计学意义( P= 0.000) 。超声造影诊断甲状腺良恶性结节的敏感性为87.8%,特异性为88.8%。
结论: 超声造影可以显示甲状腺结节内部及周边的血流灌注情况,对于甲状腺结节良恶性的判定,具有重要的诊
断价值。

[Abstract]
Objective: To investigate the value of contrast-enhanced ultrasound in the differential
diagnosis of benign and malignant thyroid nodules.Methods: There were 78 people in our hospital for
tests were found to have 95 thyroid nodules, the 95 cases of solitary thyroid nodule were enrolled in this
sduty from March 2014 to March2015, these solitary thyroid nodule ultrasound contrast images and conventional
images were retrospectively analyzed,and observe enhance uniformity、rim enhancement.Compare
grouped according to nodule diameters and nodule nature to observed nodular enhancement features.
Results: In the 95 cases of nodules, these were 42 malignant nodules and 53 benign nodules.Enhancement
uniformity, rim enhancement were proven with significant difference between the benign and
malignant nodules( P = 0.000) .Between the groups divided by diameters, the benign and malignant nodules
groups were proven with significant difference( P = 0.000) .Between the groups divided by nature,
benign and malignant nodules groups were proven with significant difference( P = 0.000) .The sensitivity
of ultrasound contrast imaging in diagnosis of benign and malignant nodules was 87.8%, the specificity
was 88.8%.Conclusion: Ultrasound contrast can show blood flow in and around the thyroid nodule perfusion,.
The ananlysis of enhancement characteristic is valuable in differtial diagnosis of benign and malignant
thyroid solid nodules.
